Cost-effective.
This fully insured prescription drug plan is designed for companies with five or more employees.  Premiums are based upon group size, employee ages, occupations, location and employee gender.
 
Coverage.
Groups with 5 or more employees are guaranteed issue and premiums are guaranteed for 12 months.  Please ask us about special participation requirements and rules that apply to groups of 150 or more employees.
